In a high-sided pot, heat up the milk with a pinch of salt. When it starts to boil, sprinkle in the polenta. Season to taste with black pepper and nutmeg. Lower the heat and stir until thickened. Then stir in the butter.

Lightly oil a baking tray and pour in the polenta. Smooth out until even and 1 1/2 centimeters thick. Set aside.

Separately, in a heavy-bottomed pot, heat up a drizzle of olive oil. Add the garlic and 3 large leaves of sage. Stir and quickly add in the tomatoes, sugar, balsamic vinegar (optional), and salt and pepper to taste.

Lower the heat and let simmer for 20 minutes until thickened. Keep warm.

Turn on the broiler of the oven.

Using a knife or cookie cutter, cut the polenta in pieces and place them on the baking tray lined with parchment paper. Sprinkle them with Parmesan cheese and black pepper and broil until golden.

Remove the sage from the tomato sauce and plate the polenta on top of the sauce. Garnish with sage leaves.
